For Family or Friends

We work in collaboration with many health professionals and regularly receive patient referrals due to the high quality provision that we provide to our clients. We consistently receive positive feedback from our service users who praise us for the service that we are able to provide for them. Our referrals come from healthcare professionals such as case managers, commissioning managers, solicitors and clinical neuropsychologists. Referrals can also be made by family members and by potential clients themselves.

Our referrals team provide information and advice on the services we provide and how they can be funded. We can send you our welcome pack and even visit your family member or friend to talk about how we are able to support them.

If a referral is made on behalf of a person with an acquired brain injury, neurological condition etc; the referral will first be confirmed with the person being referred before any further arrangements are made. This is to ensure confidentiality. We will require consent from the person who requires support before commencing any service. If the person who requires support does not have capacity or is unable to provide consent we will require consent from the individuals legal power of attorney (hyperlink – https://www.gov.uk/power-of-attorney).

We accept written and digital referrals; these should include essential clinical information, personal information about the potential client, the reason for the referral and a contact number. A member of the referrals team will assess this information and will contact you to discuss your requirements further.

If you know of a family member or a friend who you feel requires support in their own home or out in the community, please contact us.
